SEO settings of posts change custom post

When i add new post all seo settigns come default from one of my old post. After i insert my includes to post and finish it i change SEO setting from below of the post page. Till here there is no problem. But when i go into any post there is same default SEO settings again. And if i change anything includes of post and save my seo settings goes and default settings comes again. I tried many settings and i get crazy, please help. (sorry for bad English)

No. Regarding to your question i disabled all extensions and problem continue.
When add new post it comes with some SEO includes (as you see screenshots)…
