I use AIOSEOPro on http://atmac.org/ very successfully but recently all of my media attachments have been showing up with an empty page title tag. In the html HEAD section the I see the opening and closing <title></title> but there is nothing between the two.
The title is correctly displayed in the H1 tags at the top of the page and correctly shows up as title in the media editor on the WP backend.

Upon experimenting, I can reproduce the effect using the default Twenty Fourteen theme and with only the AIOSEOPro plugin active, and nothing else, so I’m guessing this may actually be a real AIOSEOPro Bug. If I deactivate AIOSEOPro the title is correctly displayed as “Attachment Title | Blogname”, upon activating AIOSEOPro the title goes back to being blank in the page source (browsers generally display as the URL of the page in the title bar when this happens).
Current versions are WP 3.9.1, AIOSEO 2.3.1. It was definitely present in AIOSEO 2.3 and *may* have been one or two point versions earlier – I don’t watch super closely.

SEO for custom post types is currently disabled.
I have tried enabling with the following settings in the “Custom Post Type Settings” box, as per your instructions:
– SEO for custom post types = enabled
– Enable Advanced options = enabled
– SEO on only these post types = Posts, Pages, Media all checked
– Custom Titled = checked
– Media title format = %post_title% | %blog_title%
This has not made any changes to title tags which are still empty.
I also tried with one media attachment setting as follows:
AIOSEO > Main Settings > Title = Test 1 2 3 test
and this title DID appear correctly – here: http://atmac.org/voiceover-in-…..e-voices-2
But after clearing caches and reloading browser windows, all other media attachments still have blank titles, e.g.: http://atmac.org/blueant-s4-ha…..unes-radio and all the examples from the first post.
I am not keen on the idea of manually copying every attachment title into the AIOSEO title window, though it would be an effective workaround, but please let me know if you have a better idea!!
